Athletic Physicals2009/2010 Athletic Physical Clearance Information: For the 2009 / 2010 school year we will be using a new physical exam process which will give you flexibility in scheduling. Instead of holding a one-time physical night Aliso Niguel will be partnering with South Coast Medical Group in an effort to provide you with a low cost alternative physical exam location.
When you visit South Coast Medical please remember to take our schools forms with you. These are the only forms we can accept as they are approved by the CUSD. In addition if you can't attend the physical with your son / daughter you can fill out and sign a consent form from South Coast Medical and they can complete the process without you. Instructions for Completion: Please make sure that you have downloaded all 6 (six) pages. You can find these forms under the section marked "athletic forms". Cover page with direction. This does not need to be returned Page one will be completed by you and the physician. Page two will be completed by the parent / guardian Page three is the CUSD Athletic Code of Conduct. Both the student and parent/guardian must sign this section after reading. If you have any questions please feel free to e-mail me and I will be happy to answer. Page four is for medical insurance verification. A current copy of your son/daughter's medical insurance card must be included and attached in the space provided. Page five is the Consent to Treat in case of an emergency. Please sign and date after reading. |
